But I also think "good"/"bad" here is a spectrum rather than a binary and exceptions to the rules always exist

Not a dev. I think "bad design" is anything that is counter to the overall design goals of the project. So, if you're designing a game that's about forward momentum, including elements that stop the player dead is "bad" design. If you're trying to make a relaxing game, a countdown is probs bad. etc
