I don't know if this is generally the case but it seems to me maybe we haven't fully moved on from the idea of having lots of LSAs around (I never have any). Whether they were effective is a different q, but at least you could claim you were being inclusive while teaching for the majority.

Agreed. More useful than SEND training in my general experience is time and support spent developing better and more inclusive curricula and resources.

Can you explain what you mean by this? Or point to some particular reading/research etc? I am unclear what expertise you are referring to.
In general do you think the best approach is that all learn together in a similar way? And that adaptations are generally made only for those with significant impairment e.g. disability/very weak literacy?
I mean because we don't often get to speak to persistent school refusers. I'm sure there are examples of poor systems etc, but pretty much any teacher I can think of would just be happy to see their students in school.
I think this is how most teachers would respond to students who school-refuse, given the opportunity.
