I describe Hyve as transmasc nonbinary and attracted to masculinity. They were mainly inspired by the idea of the Rebis - an end product in alchemy, represented by a being with both male and female qualities. They were taking an alchemical symbol literally but Boils was like "Huh? Sure, go for it."
He can recommend a great Menzoberranzan leatherworker for all-terrain gear. And also knows the best boutique for silk sleepwear.

Apothecary Upstairs as a flat floor plan. Hyve: Feels like it’s… They place the end of the stethoscope between Famine’s bicep and inner elbow. A “thump” is heard. Hyve: ... here. Thump thump... thump...

[3-4/4] In my headcanon, blood pressure is a fringe concept in Faerun. Amidst all the magical healing and diagnosis methods, the importance of blood pressure has yet to be standardized.
An engineer was onto something with this sphygmomano design which caught Boils' attention at a med convention.
Famine returns to the shop with another basket of bread. Famine: I’m back. Hyve: Hey, Famine. Boils: Famine. Famine: Hyve, Boils. Famine: Oh, Boils, before I forget, Mrs. Mathilde wrote a lo- Boils: Now that you’re here... Lad, get the sphygmograph. Hyve: Yes, Doc. Famine: The what?
Boils: It’s a device for measuring blood pressure. Noninvasive, I assure you. No more tubes in the arteries. The shop could offer readings with this and Hyve’s getting a good grasp of the method. Famine: Alright, let’s see it then. Hyve: Thanks, sir. Please take a seat and roll up your sleeve.

Famine walks down the streets of Baldur's Gate with a basket of bread. His arrival in the apothecary is automatically signaled by a bell. Hyve: Famine! Welcome back, sir. Famine: Hyve! Fronting the shop today? Hyve: Yup. Dr Boils is downstairs extracting his own venom. A panel shows wildshaped Boils as a snake. His fangs pierce the fabric lid of a jar that's collecting his snake venom. Famine: Don’t tell that to the customers. Famine offers his bread basket: Have you eaten breakfast yet?
Upstairs, Famine rummages through what should've been the kitchen counter. Famine: Where did Boils leave the coffee this time? Hyve: It’s on his study sir. He stayed up all week. Hyve grabs a bun and eyes Famine’s worn coat hanging on his chair. Hyve: Uhm... would you like me to mend your coat, sir? I read up on sewing techniques - on fabric, I swear. I could hem and patch.. I promise it won’t look ugly. Famine: Oh? Haha… I don’t need it to look pretty either. If you can make it last a few more ventures, then sure. Hyve: I-I’ll do my best! It’ll be ready before the next time you go.
Famine is Boils' delivery and errand guy. He didn't work on a schedule though, and had gone on monthslong pilgrimages often. Those periods shortened so he can do his part in caring for Hyve.
When Hyve's homesickness didn't hurt as much anymore, they took their chances at reciprocating that care.
The allegations that Boils only brought them to gnome boutiques for clothes shopping are greatly exaggerated.
Famine: Hyve, what are you wearing? Hyve: My shirt was getting short so I extended it. An arrow points out how they wear their apron: Apron skewed to the side to add personality. Famine to Boils: Shopping money, now. Hyve: It's ok, really. It's just clothes.
Boils used to buy new clothes for Hyve. Eventually, Hyve didn't want to impose when they could repurpose old fabric that would get stained or burnt off anyway. While Boils approved of the resourcefulness, Famine didn't understand their fashion sense.

Living with Dr Boils was a cycle of testing safe treatments or routines, then repeating one that works until their dark urges fight back. What he didn’t figure out was that Hyve’s not a garden-variety bhaalspawn.
