The Cult of the Eye worked for years to bring back to life one of their demigods. But their ritual went wrong. And the Eye Tyrant came back to life in a state they could not foresee.

That's why I love the concept of "Sure, you succeed. For a price"

(A small amount of damage, they lose a piece of gear, the group gets separated, Disadvantage on their next check, they get Poisoned for 1 hour, etc).

This keeps the adventure flowing.
