For the curious, C# code was calling C library code with an async C# callback to our code, but the C library code didn't invoke the callback correctly and caused the process to momentarily freeze until the callback completed, which was no big deal in normal load but under heavy load broke it.
Just experienced the opposite of "embarassing thing you did years ago", when I inexplicably remembered an impossibly complicated bug that involved an entire web service deadlocking under instense load that I solved years ago.
It's a good feeling. Wish it happened more often.
