trying to highlight that avoiding definitive treatment for low risk cancer is only part of the challenge of overtreatment - we also need to consider the host - and avoid futile toxicity in men unlikely to benefit from definitive treatment due to their multi morbidity...

guidelines require life expectancy estimates. We show (on population level) using a validated tool to estimate LE, that pts with limited life expectancy are increasingly being treated. (e.g. intermediate risk #prostatecancer and life expectancy < 5 years)
Works as well as for intermediate / high risk disease too... The biopsy confirmed that you have cancer, but we can still prioritize other aspects of your health... or lets monitor this cancer because initial treatment may worsen your quality of life...
