As I understand it, there are no such thing as 'wrong forms' . We are not contractually obliged to use any specific forms for referrals. The using of 'specific forms is a choice we have if we find them helpful.

GP life is getting tough. This week I spent 30mins each trying to reassure
1. One man that his ECG was normal (despite the digital interpretation)
2. Another, fit and healthy chap, what a Qrisk3 of 13% actually meant to him
Both insistent they wanted a private referral to cardiologist to discuss.
