It may be a mix of public & private institutions with their hooks embedded in the state. Obama was seen as an outsider to fix it on America. So was Trump. Right wingers call it the deep state or The Blob. Left wingers call it failed government. Again I don't know the answer in general.

I don't know what would fix it. Also I recall previous PMs including Tony Blair discussing it. Their answer was to apparently be extremely switched on as PM. Possibly only Thatcher & Blair that had the presence of mind to be able to deal with it.
