I personally consider Lethbridge-Stewart and Sgt. Benton companions mainly because they've traveled in TARDIS. Both in The Three Doctors, and Alistair from 2 different time periods in Mawdryn Undead. Plus Alistair was taking by Time Scoop with the Second Doctor in The Five Doctors.

I think the Doctor only uses it as a last result and refuses to consciously choose a new face (like when the Second Doctor refused to accept any of the choices shown by the Timelords). Eight becoming The War Doctor (in Night of the Doctor minisode) is the only time he chooses to regenerate.
