Delighted that @ukhsa.bsky.social has played a pivotal role in developing and synthesising evidence to support the national vaccination programmes for gonorrhoea and mpox. This is in addition to hepatitis A and B and HPV. 1/2

My fav take home message: vaccination is key. Modified Vaccinia Ankara vaccine 2 doses > 80% effective. Protection limited in post-exposure setting ~ 20% within 4 days of exposure. Need to focus vaccination on those with ongoing exposure #BIASpring2025

Helpful review of tecovirimat RCTs for Mpox...No benefit over placebo. Raising basic standard of care however in all settings could make a difference. Therapeutic options limited #BIASpring2025

Thanks to Catherine Houlihan @ukhsa.bsky.social for an interactive update on Mpox @biainfection.bsky.social trainees/clinical day. Key practical messages:
HCID units/PPE no longer required
24hr testing no longer justified BUT
Still notifiable
Still need to prevent transmission #BIASpring2025

My post below feels so last year..all the questions in the first session were asked via Slido. It feels like this should be a game changer in terms of who asks the questions. Remaining anonymous probably also helps so we may never know. #BIASpring2025

Was the 'end of global health' writing on the wall #BIASpring2025. Reliance on a single donor i.e. US is a key issue. We need to rebuild differently (and straight away!)
Great Q&A session with Prof Joe Jarvis

#BIASpring2025 Prof Joe Jarvis: What can we do?
-Lobby our government
-Document what is happening
-Keep talking about 'silence = death'

But no clear way forward...

The end of global health (as we know it). Prof Joe Jarvis opening #BIASpring2025
highlighting the devasting impact of US funding cuts on global health. The issue is not a lack of money just how it is spent as described by @drtedros.who.int 👇🏾

Staying when it is not working creates a smoke screen that could prevent meaningful change. Not an easy decision as an individual as leaving is for the benefit of the greater good over individual opportunity.

Could we move towards a model more like the childhood imms model, and make and schedule standard imms appointment at age 60 or 65 to receive all recommended vaccines at one appointment? Why not make it a general well being check up to add value for time. #IDSky #vaccines

Final point in this 🧵was going to be about how combination vaccines fit with current policies. UK recommendations for vaccines for older adults vary by age 60(shingles, but staggered intro from age 70, 65...) 65(flu, pneumo), 75( RSV, COVID-19) and a need to increase uptake of nonseasonal vaccines

Still to be answered
does higher antibody concentration
= improved efficacy, and effectiveness?
= in theory should mean longer duration of protection but remains to be seen
=how do we lessen the reactogenicity? - we need studies with prophylactic paracetamol
