Multicountry genomic analysis underscores regional cholera spread in Africa - Nature Communications Cholera remains a significant public health burden in sub-Saharan Africa, but the mechanisms of continental and regional spread remain undefined. Here, the authors investigate recent patterns of spread using Vibrio cholerae genomic surveillance data collected by a consortium of seven African Union member states from 2019-2024.

A strong illustration of the importance of highly collaborative efforts and multicountry solutions to cholera control. Coordinated genomic surveillance can uncover cross border transmission and critical shifts in the bacterial genome.

Tuberculosis: Documenting the transition to shorter treatments for multidrug-resistant TB under real-life conditions In Pakistan, Epicentre is conducting a study to better understand how new, all oral, shorter treatments for multidrug-resistant/rifampicin‑resistant tuberculosis (MDR/RR-TB) are implemented in real-world settings. These treatment regimens have already proven effectiveness in clinical trials, and are recommended by the World Health Organisation (WHO), but what about once they leave the controlled setting of a trial? The study explores their impact on patients' lives, their feasibility for healthcare providers, and their acceptability in a context where challenges remain numerous: treating children and pregnant women, managing highly resistant forms of TB, and monitoring the emergence of new resistance.

New shorter, all oral MDR/RR TB regimens work in trials, but how do they work in real life?
A study in Pakistan looks at feasibility, acceptability and impact of MDR/RR TB treatments beyond controlled settings, including for children and pregnant women.

Effect of COVID-19 pandemic on ART access and timely initiation in people living with HIV in 31 countries: a regression discontinuity design study Objectives The COVID-19 pandemic threatened global HIV Test and Treat Efforts. We assessed whether it affected (1) the number of antiretroviral therapy (ART) initiations and (2) the proportion of time...

Most HIV programmes kept ART initiations stable during the 1st year of COVID 19 — a sign of resilience. But declines in some settings show the need to understand what sustained ART under strict measures so we can be better prepared.

Low-dose yellow fever vaccination in infants: a randomised, double-blind, non-inferiority trial Compared with the standard yellow fever vaccine dose, a dose of 500 IU did not meet the non-inferiority criterion, suggesting that minimum dose requirements in adults are not generalisable to infants....

Yellow fever & minimum vaccine doses
500 IU works in adults — non inferior to the standard dose.
👶 In infants, it’s different: a Kenya/Uganda study shows 500 IU fails non inferiority.

Diagnostic Performance and Kinetics of Hepatitis E Viral RNA and IgM Antibody Test Positivity in a Genotype 1 Outbreak in South Sudan The assure IgM RDT showed higher sensitivity for identifying hepatitis E than PCR and similar specificity to IgM ELISA, supporting its use in surveillance.

Using surveillance data from MSF’s Bentiu vaccine study, we evaluated the performance of the tests. The Assure RDT showed good sensitivity (86%), outperforming PCR and specificity comparable to ELISA (94.7%).
