It turns out, that under the right conditions, carbonyl groups form on MPO. Hydralazine actually ATTACHES to these carbonyl groups, probably modifying the MPO conformation and inducing a neo-antigen, which eventually causes the formation of anti-MPO IgM!

Most SEs from steroids are longer term and cumulative, aside from infection. I don’t know of many folks that would rx a CNI first.

Steroid if it’s their first or second go, if frequently relapsing go next to CNI, then if breaking through CNI and prednisone it’s RTX
