ICU Hemodynamic Secrets:
Whenever I manage a critically ill patient, I am interested in knowing the blood pressure & at least an estimate of the cardiac output (CO). Since placing a pulmonary artery catheter takes some time & effort, I am usually employing workarounds based on #POCUS:

This week took us to ATLANTIS (trial, not the lost city)! This observational study looked at impulse oscillometry in asthma, and found high small airway resistance (5Hz—20Hz) had a 2.26 HR for exacerbations. Small airway dysfunction was found in 36% of well-controlled patients.

Short of breath with CTEPH? Our latest read found continuing riociguat for 16 weeks after ballon pulmonary angioplasty (BPA) showed no loss of mean exercise cardiac index, compared to a slight loss seen in placebo after BPA. Also, no increase in adverse events with riociguat!
