It's a beautiful piece, powerful and poignant - thank you for writing it.

All good wishes to you and your family.

(I tried to tag you when I posted it but wasn't able to find you, apologies.)

Sobering talk from Timothy Walsh from Oxford, UK on the rising antibacterial resistance even to the NOVEL agents

For example we are already seeing growing reports of Cefiderocol resistance - and we have barely started using it.

As usual we are being outrun by the bacteria

"💬 Viewpoint: Mandatory annual training modules consume millions of physician‑hours each year, yet their largely passive formats may offer limited educational value while contributing to administrative burden and clinician burnout."

Oh, absolutely, and I'm grateful for it. Think I'm still v. tired from a very busy Friday at work last week.

Ugh, struggled on my #run today.

Too hot, got clothing wrong, had to stop twice, pacing all over the place, felt like a slug in treacle.

Some days are like that.

Onwards...

Screenshot of text:

The non-technical elements, particularly discussion of risk, shared decision-making and management of uncertainty, become a much bigger part of your job, and can use a lot of cognitive bandwidth.

There is nuance in most situations.

The 'hive mind' can be invaluable - don't be afraid to sense-check your thoughts and plans with your colleagues - or to ask for a colleague to stand by in case a second pair of hands is needed.

Be aware of your own behaviours - for example, I know I can sometimes become quiet when I need thinking time - it's useful to let colleagues know that I'm not ignoring them (!) but it's the way I manage my thoughts before vocalising them.

Remain humble - junior colleagues may have more experience in something than you do - learn from them.

Allow yourself a few months to settle in before taking on other departmental or organisational responsibilities.

It takes time to get used to the additional responsibility - it might feel uncomfortable or overwhelming sometimes - that is normal - allow yourself some grace.

Trust your gut instincts, they are the result of many years of training and experience.

Some thoughts I have jotted down after a few months in my consultant post.

Our department is brilliant for this, it's one of the reasons I wanted to continue to work there. And another advantage of anaesthesia being the largest hospital specialty 😆

Gosh yes, it must be hard being the sole specialty consultant!

Sticky toffee pudding and vanilla ice cream, with a cup of Espresso coffee in the background

Today was one of those days at work when I felt particularly grateful for colleagues - people to 'sense check' with, and share some cognitive load.

Colleagues are everything in the NHS.

The day ended with a meal in a village pub, topped off with sticky toffee pudding and Drambuie.

How do you make it?
Was just wondering recently if it was possible to cook with wild garlic, there's so much of it around near us.
This looks fabulous 🙂😋

Similar issues in Scotland- why are dying patients not getting the care they need paid for? Why does the NHS pay for obstetrics but not palliative care- we are all born and we all die- it shouldn’t rely on charity to provide that care

From early in medical school, I never really understood why General Practice was often portrayed as the 'easier' career option

The level of cognitive demand, intensity, and systems & individual overload, seems higher now than ever

I honestly don't know how GPs do what they do

All respect to them
