Whether the administration has the authority to cancel aid approved by Congress is now the subject of legal pushback. But for global health experts—and for cities like Los Angeles—the deeper concern is how fast a virus can cross an ocean when the world’s defenses are down.

The government has confirmed only $8.3 billion in unobligated aid remains, down from the $40 billion USAID once administered annually.

California has seen its own scares in recent years.

Some global health funding will continue, including HIV and tuberculosis programs and food aid to countries facing conflict and natural disaster. But the scale of the retreat is hard to ignore.

The administration is also cutting support for the UN Food and Agriculture Organization’s disease monitoring network in 49 countries. That system plays a quiet but critical role: tracking animal-borne illnesses like bird flu before they make the leap to humans.

As global coverage weakens, the chance of a case landing in Southern California only grows.

Dr. Sania Nishtar, CEO of Gavi, says it’s not just about protecting children in faraway places. “Every vaccinated child makes the world safer—including for Americans,” she said.

For Los Angeles—a global travel hub—those risks are closer than they seem. LAX sees tens of millions of international passengers a year, many from countries where vaccine-preventable diseases still circulate.

An estimated 1.2 million preventable deaths.

“This is not just a bureaucratic decision,” said Dr. Austin Demby, Sierra Leone’s health minister. “Children’s lives are at stake. Global health security will be at stake.”

A 281-page spreadsheet sent to Congress lays out the cuts in bureaucratic detail. But for health officials, the implications are immediate. Gavi estimates that without U.S. support, 75 million children could miss routine vaccinations over the next five years. The human toll?

Also at risk: malaria prevention programs and disease-tracking efforts in dozens of countries where outbreaks often begin.
