i have for example a separate account for interacting with nsfw. others have a personal and a private one. either way, you might end up interacting with the one you last used instead of the one you enabled notifications with.
#bluesky finally added the ability to have notifications on what somebody posts, but please beware of an issue with it.
if you use multiple accounts and have notifications enabled, it will not switch to the account that you enabled notifications with. so you might like or reply with the wrong one.

I also have my own issues, including depression, stress and a pretty severe degree of dysphoria. Some of which I can get help for, others I'm already in queue for but will still have to wait for several years. Being told I should consider myself lucky is not particularly sympathetic either.
