New case report published on trigeminal ganglion stimulation for refractory anesthesia dolorosa.

Rather than a binary “complete anesthesia + pain,” clinical nuance suggests a spectrum of deafferentation.

electrodes can stay forever though like anything they can slowly degrade over time! We have some patients with electrodes in their brain for more than 20 years!

And it isn't just about invasive stimulation. It is a tool that we have amongst many and has its uses. But at the same time it sets the stage for validating noninvasive stimulation approaches. We can do invasive stimulation for those who truly need it. #depression

This is the future. The reality of biological #psychiatry. We listen to the patient, create a personalized map of their brain, use algorithms to disrupt stuck networks, and adapt the stimulation over time while continuing our quest to understand those networks.

What is really interesting is that these networks are embedded in the same networks that play a role in executive function and working memory as well as emotional regulation. @alexanderbherman.bsky.social

We have always known that there is the possibility of top-down modulation of pain circuits potentiating the endogenous analgesia network. The question has been how do we even think about trying to target. PFM seems to be a giant leap forward with @drdamienfair.bsky.social @ndosenbach.bsky.social

These patients and many others suffer from central neuropathic pain that often is due to #stroke or #multiplesclerosis or treatment of #trigeminalneuralgia resulting in #anesthesiadolorosa. These are some of the worst pain syndromes on earth!

With a thousand people essentially choosing between slot machines, we were able to figure out that anxiety traits and apathy traits provide counterbalance pushing us to either search for better options or stick with what is working, guiding our decisions #universityofminnesota
