Sometimes it's to put some urgency in the players, so they aren't strategizing for an hour out of game in 5 minutes in game. I also try to randomize it too, so it feels less gamey. My players have reacted well to it, the pressure frequently brings out some interesting ideas.

Sometimes there is, sometimes something that seems inconsequential happens, sometimes nothing. I try to have things that make sense happen, like a band of ogres isn't going to show up and attack if there isn't a band of ogres nearby. But a guard moving to a different position? Sure.
I do this frequently any time my players start planning anything. Sitting outside the bandit camp planning an ambush? Timer. Trying to break into a Noble's house and are trying to decide how to enter? Timer. A great tool to make everything feel more tense, even if nothing is actually attached.
