Case 22-2024: A 30-Year-Old Woman with Postpartum Fever, Abdominal Pain, and Skin Ulcers | NEJM A 30-year-old woman was admitted 6 days after the birth of her first child because of fever and abdominal pain. New skin ulcers were noted. A diagnosis was made.

8. Visual Hallucinations - Psychosis Association: Visual hallucinations more often linked to drug-induced than primary psychosis. Fun fact: 1 in 10 adults are afraid of clowns! Reveal yourselves. PMID: 37922514

7. Strep Bacteremia & ID Consult: ID consult in strep bacteremia cases lowered mortality (NNT=18). PMID: 38525878

6. Pip-Tazo vs. Cefepime in Sepsis: Pip-tazo was associated with higher mortality than cefepime in sepsis (NNH=20, retrospective). Theory: Pip-tazo impacts gut anaerobes. Consider pip-tazo for anaerobic/intra-abdominal concerns. PMID: 38739397

4. Steroids in Severe CAP: In severe CAP (>6 LNC), steroids (hydrocortisone 160-200 mg daily for 7 days) offer mortality benefit. PMID: 37076606
5. Patient Satisfaction & Team Sitting: Adding a chair at the bedside nudged teams to sit with patients, increasing patient satisfaction. PMID: 38101923

3. DC Summaries - PCP Preferences: PCPs prefer discharge summaries to include: to-do lists, incidental findings, justification for medication changes, and duration of treatments. Day-to-day notes can be omitted. PMID: 38010464

1.⁠ ⁠LR vs. NS in Acute Pancreatitis: LR shows better outcomes than NS in acute pancreatitis (meta-analysis). Theory: LR suppresses inflammation. PMID: 38015322
2. UA & UTI Diagnosis UA with <20 WBC/hpf has a good NPV for UTI (but low PPV if >20 WBC/hpf; symptoms are essential). PMID: 38666412

Iron deficiency is always caused by something.

🥩inadequate intake
💫impaired absorption (celiac, IBD, bariatric surgery, meds, foods)
🏃increased needs (vigorous exercise, pregnancy)
🩸losses (menstrual, GI)

Treatment should always involve finding AND addressing the cause.
