We had our first palliative liver MDT today 😊

Several months in the planning, thanks to the residents who challenged their seniors at an m&m for delayed recognition of dying

I’m not sure where I’m fitting in the extra work generated, but these patients can really benefit from pall care so I will

Learning CPR on manikins without breasts puts women’s lives at risk, study finds Analysis of training models on global market found all had flat torsos with just one having a breast overlay

“CPR mannequins with no breasts are emblematic of healthcare practices being optimised for the white male body; with detrimental and sometimes deadly implications for anyone who doesn’t fit this mould.”

Prognostication… an art not a science.
How often is this also shared with the patient and their family too?

As a GP, I initiate conversations about limited life expectancy when appropriate. They are less initiated by secondary care teams and patients sometimes disagree with me- ‘my consultant is trying to build me up’. An important topic for discussion particularly for secondary/primary care interface.

The SHIPS Study: – SHaring Information at the Primary / Secondary care interface

4. Further research required to understand why this info isn’t shared more frequently, what the info needs of primary care clinicians are, and to capture patients’ views and experiences. Find out more here: ships.blogs.bristol.ac.uk

3. Secondary care clinicians should be supported and encouraged to share info regarding prognosis with primary care.

2. We suggest that a review by the #pallcare team during an admission should always be communicated in the discharge summary.

TL:DR -
1. Communication of poor prognosis from secondary to primary care doesn’t routinely occur, but GPs value it.

Hospitals rarely provide information to GPs about poor prognosis, research shows Information about poor prognosis is rarely communicated from hospitals despite this being vital information for GPs, a study has found.

Hospital teams rarely share information about patients' limited life expectancy with GPs, even though GPs find this very helpful in planning patient care.

The uncomfortable truth about assisted dying Throughout all the questions, there is a baseline assumption that assisted dying will likely mean a smoother and calmer death. But does it?

Uncomfortable and important article by Prof Mark Taubert around what an assisted dying death actually looks like. For many it is worse than the natural death the vast majority have now. These peaceful undramatic deaths have been lost in the polarised rhetoric.

Over 80% of locum GPs in England are struggling to find work YouTube video by Channel 4 News

Can’t get an appt with a GP?

Yet there are 1000s of GPs out of work or struggling to find work
NHS England have cut the equivalent of 2 full-time GPs in funding from an average practice, whilst funding ‘additional’ roles via ARRS

£2bn cut in real term

Not additional then🤷🏻‍♂️ youtu.be/tI69wqP1_3g?...
