I don't think there was any additional cost. But these incidents span from the late 90s to 2009. I wouldn't be surprised if it's a worse (and more expensive) experience now.

FWIW I believe I've missed flights thrice. (Once as part of a slow-moving group, twice by mixing up the departure time.) First time they put me up in a hotel for early departure the next day b/c it had left earlier than scheduled (or something?), the other times I just had to wait for the next one.
