To me - depending on the game - either only Intended or Intended and Story is good. Maybe "intended+" for the second playthrough that does something more than just adding zeroes to everything. With the game being designed around regular intended.
Custom difficulty could work too, depending on a game
Experience of preparing and building up the player for bigger challenges that involve more and more aspects of the game in every encounter - that's achievable in a single playthrough too.
I'm not sure no deaths no heals run on the 2nd/3rd playthru being the "real" run should be the starting point
RE games are short and you expect them to be replayable already. Designing around replayability with the last difficulty being "the ultimate test" of your understanding of everything the game has to offer is certainly great if you're making a RE game.
Idk if it will work for other types of games.
I do agree that designing for your main experience to be the middle ground between the intended (but too much friction for some players right away) and the story mode would make for a way less compelling game overall.
You mentioned RE9. Imagine it was designed around Insanity from the ground up. This game would be completely different at best and suck absolute ass at worst.
And that already if we assume game at its most challenging is a game at its best. Which IMO is questionable even for the genres that you expect a challenge from.
Wouldn't that create the same problem? Because unless you go straight to the hardest difficulty right away or replay the game several times - you won't have the "full" experience.
Some genres might work, but then again at this point why not have the "intended" difficulty to be the only existing one?

For one of the Distant Eden (character) outfit commissions I also had to design this sword with a fancy guard. The client's friend is a blacksmith and they actually made a real sword based on my design!

Which is not only makes it difficult to build audience that engages with your stuff in a more active manner, but it's actively hostile to doing so.
Another big contributing factor is all the cool places on the internet USED to be different places all over the web, but now most of everything is consolidated into just a couple of platforms.
Even without algorithms you end up with single digit number of feeds for literally everything everywhere.
With communications in mind, just doing your own research, figuring out designs and landing on something you and the client both like would probably take a month alone.
Then inevitable revisions and at least one "if only we made this change during sketches" moment that would eat another week ahaha

It's great, one of my favorite modern shounens.
I don't engage with fandoms much tho (as a rule of thumb for like everything :D)
And I'm also counting in ideal vacuum conditions: no distractions, no other obligations, no life events and crises happening, prime artist form and so on.
Which is very unlikely in real world.
BUT the major factor for me will be the communication. Because, especially on bigger productions, I had projects I've done drawing for in half the time it took for feedback to go up the chain of command and back.
So for for my own project: yes PROBABLY (but idk)
For commercial work: no way in hell
Assuming at least five 6-8-hour workdays a week, incredibly detailed brief with fully figured out designs and a full month salary for you to do only that with zero revisions - that's 120-160 hours of raw drawing time minimum. I think it COULD be feasible.

I’m an illustrator and concept artist who works primarily in fantasy and horror. I’ve done work for Magic, Dungeons & Dragons, Centipede Press, Draco Gaming, Arkhane Asylum, and more.
