Single/double loop learning theories are great but I think there is sadly I feel there often ease of un-learning too in memory.

Even when your brain is full and you think academic supervision will just add to that fullness… it’s totally what is needed to strip your thoughts and ideas right back. Obviously I have an amazing supervisory team who I know are running the race with me. Never skip supervision!

LFPSE - weekly thought - how can we learn from incidents when the rates are still love and every and the shift from ‘other’ cause category to ‘not reported’ has increased. It is still very worrying!

Think I’d add the need to consider organisational and individual un-learning theories here… it’s the layering of add ons (sticky plaster) without striping back and giving permission to forget (safely) and rebuild - with rationale embedded. Idealistic maybe!
