Being an OCD-type anaesthetist my approach is a single-ply tissue corner on the skin so that just folds on touch and repeated single touches (no spatial/temporal summation) with no visual or audible cues along the anterior or mid-axillary line until an affirmative ‘yes’ from the patient. Too fussy?

Bouncing makes no sense if using saline. Bouncing is done intermittently with incremental needle advancements. The next 2-3mm advance may = DP. Continuous advancement needs only Huber rim to exit ligament for LORsaline (test it out on a simulator) It’s the Doughty technique. doi.org/10.1111/j.13...
