He doesn't exactly make the best first impression...
The music change and Denji's blood pooling on the ground as the kiss breaks in the movie lets you know right away that something is wrong, but in the manga you don't really realize what's going on until you turn the page and see that panel.
Something I kind of disliked about how the movie adapted this scene is how the tone shift happens a bit earlier. The shot of Reze with Denji's severed tongue in her mouth functions like a jumpscare in the manga, but this isn't the case in the film.
