Based on the pharmacokinetic studies I’ve read with oritavancin, I doubt it would have an issue staying above that MIC, one 1,200 mg dose seemingly gets to 100 mcg/mL in the serum as a peak. Those who actually use oritavancin (I haven’t) are free to correct me though! doi:10.1007/s40801-020-00188-6

Not necessarily, but compared to other isolates identified by EUCAST, 0.125 mcg/mL is a higher MIC than the majority. It would depend on the site of infection and if > 0.125 mcg/mL concentration of oritavancin can get there adequately.

There’s no direct ECOFF on EUCAST for oritavancin for E. faecium, however out of 320 observations, 41 had an MIC of 0.125 and only 8 were above that MIC. I’d say it’s on the higher end of comfort, but no great data for clinical outcomes.

Honestly, @empiricgame.bsky.social’s card set was great to learn some of the antibiotics. My ID preceptor (back when I was training) @ashpergillus.bsky.social played with me during my rotation and I learned a ton. Planning to implement it for my students 😝

Eep, a bit scary. Not for us since we use carbapenems as our B-lactams anyway, but I’ve been OK with TZP in uncomplicated urinary ESBL infections in the past.
