Time for another #POCUS pitfalls thread: the infamous IVC 🤩

I have MANY thoughts on the IVC 💭

When I first started teaching POCUS, I actually disliked the IVC because it was often poorly done or incorrectly interpreted ❌

I have now come to love the IVC, BUT you have to do it right!

I love love LOVE scanning the heart 😍, but when it comes to bang for your buck for ultrasound novices, lung ultrasound is such a winner 🏆

It's time for another #POCUS basics #skeetorial 🧵

Let's talk about common pitfalls in #LungUltrasound and how to correct these ✔️

There’s few things more satisfying in medicine than diuresing someone 7 liters in a day and seeing their creatinine improve.

Breaking maladaptive positive feedback loops - chef’s kiss.

Today we have more evidence that #VExUS (and not RAP) might be the optimal tool for studying congestive organ injury!

IRF after diuretics means these patients had congestive nephropathy!

The study shows that right heart catheterization is not useful to diagnose this condition. Only echocardiographic markers of RV and RA dilation, Tricuspid regurgitation and IVC size were associated with this phenotype!

Honored to lecture at the Hemodynamic Monitoring and Tissue Perfusion Course in 🇨🇱

As I was preparing the lecture, I ran into this beautiful paper detailing the hemodynamic characteristics of heart failure patients with improving renal function (IRF) after diuresis:

X 2 about portal vein 🤣

Also, not imposible to have Normal S wave IMO. Atrial relaxation also plays an important role in S wave generation. So if RA is compliant enough, it could generate enough antegrade flow.

My thoughts on this back in 2021

I have been saying this for years. I don’t really use HVD as part of venous congestion assessment.

My practice as a Nephrologist involves a lot of HFpEF with normal RV filling pattern. HVD is always normal (unless severe secondary TR).

Portal Vein does give useful info

Hepatic vein Doppler in critically ill patients: a reflection of central venous pressure or right ventricular systolic function? - BMC Anesthesiology Background To explore whether hepatic vein systolic filling fraction (SFF) is associated with central venous pressure (CVP) and right ventricular (RV) systolic function in critically ill patients. Interesting study looking at something I’ve been wondering. When looking at S vs D wave in hepatic flow, as part of #VEXUS, does a dominant D reflect high RAP/CVP or rather reduced RV systolic function (which in turn increases RAP) ?

Working on talk about #VEXUS score for upcoming #ultrasound course of Swiss Society of Nephrology.
Still no interventional trials looking at patient outcomes by using venous Doppler assessment.
Or did I miss something @argaiz.bsky.social ?
