I would also be curious in patients with respiratory symptoms if anyone is using Karius before or after bronchoscopy (if initial cultures are negative) as the AFB/fungal cultures can take several weeks to returns.
#IDSky curious how different groups are using microbial cell free DNA/NGS testing (like Karius) from a cost/diagnostic stewardship perspective. In immunosuppressed populations (HIV, transplant, ect), do you use these tests early or as a last resort (after initial testing is negative)?

That is quite a statement considering ID docs are known for sifting through ‘potential’ exposures to guide diagnostic work up. But unfortunately I agree, who knows what the near and distant future will bring with the new direction….

Ok #IDsky, helping someone with a stewardship lecture, guidelines rec 21 days for meningitis caused by GNRs, anyone have studies that actually support this. Looks like the guideline was based on historical pref and maybe neonatal data but would like something more concrete.
There are a lot of reasons - our work takes time and we do many essential functions (abx stewardship, quality, hospital Epi/IP) and do not demand adequate compensation. All that leads to long hours and ongoing questions about comp. Its rewarding work but burnout is real and trainees see that.
It is important to keep in mind that ID applications have been fairly steady but the number of positions have increased (page 55 of the Match Data report). There was a slight bump in 2021 but we have regressed to the mean in 2023. The stable candidates/increased slots mean fewer programs filled...
IDSKy seems quiet for match day. Even my IDtwitter feed has only a few programs welcoming incoming fellows (and the posts analyzing the match results). Is IDtwitter just that depleted (don't blame people for leaving) and have any ID fellowship programs moved to IDSky as their primary platform?

Agree. I joined a few days ago trying to get away from EM and X, but my X feed has been filled with IDweek stuff, can't find any of that content here yet. The few people I have found are still mostly posting on X
