Hard to say exactly, but a couple in this sense can function as one entity. So it's like two couples coming together instead of four individuals. Less coordination. An orgy feels like an event. Two couples hanging out can just decide to all fuck.

I feel like the answer lies not exclusively in numbers but more in organization. If you invite people over for an orgy and 4 people show up and have sex, it's an orgy. If two couples (still 4 people) decide to have sex together, I wouldn't call that an orgy.
