My first games job out of uni I didn't get credited on my first title because both my lead and production forgot to have me patched into the credits.
I worked on several patches that went into the game during this time so opportunity was there but I was too awkward to ask/check.
Everyone who works on a KO_OP game is added to our credits database on their very first day as part of onboarding!!! It's not hard!
