Major concern: clinical risks of positioning post-exertional malaise as a “hallmark” of cardiovascular long COVID

We read with attention the discussion article by Charlon and colleagues “Post-exertional malaise and the myth of cardiac deconditioning: rethinking the pathophysiology of long covid”. Charlton et al. present post-exertional malaise (PEM) as a hallmark of long covid and advocate for pacing strategies guided by wearable heart rate monitoring, and app-controlled progressive activity. This position raises concerns. Certainly, rejection of a “deconditioning” hypothesis in long covid is needed : as expert-patients, we know the harm it can cause. However, redefining long-covid cardiovascular involvement primarily through PEM risks undermining patient safety, especially when unsupported prevalence numbers of PEM in long-covid cohorts are cited (80%)
