Conclusions:
🔵Acceptably low primary emergence rates
🔵Lower toxicity with INO radiotherapy
🔵No OS difference vs MUC
🔵INO may be a feasible alternative in modern practice

Posts by NOTCH UK - The National Oncology Trainee Collaborative for Healt
Survival Data (see paper for full data, and sub-group analysis)
5-yr OS was similar between INO and MUC RT groups, 82% and 80%.
Death from other causes (not HNSCUP) was higher in MUC RT cohort (p=0.003).
Primary site emergence was rare, occurring in only 3/195 patients (1.5%)
Treatment and toxicity:
🔵RT dose/fractionation differed notably, with MUC receiving higher-dose schedules.
🔵Acute feeding-tube use was higher in MUC (33% vs 11%, p<0.001).
🔵NTCP for grade ≥2 xerostomia at 6 months was significantly higher in MUC group
Study Design:
Retrospective observational multi-centre study
18 UK cancer centres
Outcomes of interest: OS, emergence of primary site, disease relapse/recurrence, toxicity, and enteral feeding rates.
Study Aims:
🔵The aim of the study was to directly compare involved neck only (INO) RT versus bilateral neck and/or mucosal irradiation in patients with unilateral HNSCCUP.
Background:
🔵Management of HNSCCUP remains controversial, particularly regarding the optimal RT strategy, as traditional approaches have favoured comprehensive irradiation of bilateral neck and mucosal sites to minimize the risk of missing an occult primary.
NOTCH publication out now in @ijrobp.bsky.social !
Involved neck only (INO) vs mucosal radiotherapy (MUC) for HNSCCUP
INO RT for unilateral HNSCCUP offers similar 5-yr survival and disease control as MUC, and lower rates of enteral feeding
