Like I technically listen to my "Fans" for my game design but almost all of them are basically in the trenches with me and willing to make the effort to write up things I can process. And there's times when I just tell them no if feel I have a better idea.
One thing I have learned watching fans is that most of them don't have the deep understanding of criticism required to actually know what they really want, so what they ask for is often not even what they really, really want. Just a surface level vibe.

I think I know enough about excel macros I could make a more interesting game in excel in 6 months than these people did.
I think an ER based show might be really hard because it's hard to capture the interpersonal drama at times and the pacing would be totally messed up due to the pace of TTRPGs being quite slow generally.
To start though, you'd gamify the medical work and dealing with unruly folk.
