Besides Horton and Hodge, they all do seem training related - tendinitis, muscle strains.

Horton has had problems right from the draft on, might just be bad luck. Hodge seems got misdiagnosed at first, so that's on the docs. The result would've been the same (surgery), but that's just not ok.

Which is especially frustrating bc LLMs are probably the most problematic form of AI in terms of accuracy and scalability.

LLMs have hard limits we will hit, and AI will still exist, but the knowledge of LLMs won't transfer well to these other approaches.
