It will not be approved outside the US - plus, BAFF/APRIL are more reasonable for patients
Irrelevant drug for 7.95 billion people globally (in this indication)
No, I do not target this! Of course you want serologic remission, but it is not always achievable (and has no major clinical relevance)
I am fan of 6-monthly but only one dose
Yes, would do so; we are starting using it - Allegory data now really show the power of obi

Had a super weird case here once, but agree very unlikely! But she was PR3 sky high
IgG4?
Interesting case, indeed. PR3 in the context of C3G rather uncommon. Undetectable C3 would prompt alternative pathway investigations; also in the consideration cryoglobulins and cryofibrinogen, although C4 reduction would be more common

Absolute rarity; only once when the patient was double-positive and the MPO-ANCA became dominant!
Most will eventually turn anti-GBM antibody negative when established on dialysis! Otherwise, you might consider a dose of rituximab
If kidney is lost and no extrarenal manifestation, there would be no indication to use RTX or CYC, right?
The International Vasculitis Workshop is coming up! One of my favorite meetings for it's sense of community, and possibilities of forming meaningful collaborations (PEXIVAS was hatched in one of them). Call for early career scientists is here

It was a pleasure to collaborate with @kronbichlerlab.bsky.social and Dr. Astrid Weins on this article. We describe transformations in our understanding of podocytopathies, and novel treatment approaches in light of those advancements. journals.lww.com/co-nephrolhy...
I was also there and enjoyed the discussions a lot!

just one small note on %FPE/genetics. The concept that genetic forms have ⬇️FPE is confounded. Most FSGS related to slit-diaphragm genes presenting with NS have ⬆️FPE. We see nearly total FPE with TRPC6, INF2, NPHS1, NPHS2, MYO1E et al
