Interesting case, indeed. PR3 in the context of C3G rather uncommon. Undetectable C3 would prompt alternative pathway investigations; also in the consideration cryoglobulins and cryofibrinogen, although C4 reduction would be more common

Most will eventually turn anti-GBM antibody negative when established on dialysis! Otherwise, you might consider a dose of rituximab

just one small note on %FPE/genetics. The concept that genetic forms have ⬇️FPE is confounded. Most FSGS related to slit-diaphragm genes presenting with NS have ⬆️FPE. We see nearly total FPE with TRPC6, INF2, NPHS1, NPHS2, MYO1E et al
