Hi π. I'm Tiff Caza, a kidney pathologist and #NephJC enthusiast! Interesting topic tonight! No COI.

sat down with Dr. Salil Parikh, a radiologist who helped his facility improve their adequacy rates significantly, to discuss one possible blueprint for better renal biopsies. #nephsky #radiology

I love that. . . got an issue, get some tissue π
IgA Vasculitis
A kidney biopsy is mandatory in patients with persistent proteinuria β₯0.5 g/day for more than 4 weeks, impaired kidney function, or suspicion of rapidly progressive glomerulonephritis (RPGN).
#NephJC
When hematuria is the predominant presentation, does anyone do a genetic workup first or straight to biopsy? #NephJC
Could an increased frequency of IgA vasculitis in children be at play as they are more often proliferative? #NephJC
Great point! #NephJC
Thanks for hosting us!
When there is segmental sclerosis in an IgA nephropathy, patients may never achieve less than 0.5 g proteinuria. How does this impact the course of therapy? #NephJC
The evidence of immunomodulatory effects of steroids and Nefecon comes from the following trials. High dose steroids treatment suffered from complications like infections. Nefecon, however, still showed similar systemic side-effects of other oral steroids.

Treatment Approach: π
Novel therapies for IgAN have shifted the paradigm to a bi-pronged approach.
a) Generic responses to HTN/proteinuria
b) Fundamental drivers of nephron loss by reducing the formation of aberrant IgA molecules & the maladaptive mechanisms that contribute to CKD. #NephJC
While it may not be validated for that purpose, is there any utility in post-transplant? #NephJC
Global Implementation of IgAN 2025 guidelines:
#NephJC
As we learn more and develop new treatments, it is important to try to make these available to a wide population of patients. Including low-income patients in future studies might allow for at least some access to cutting edge therapies.
This seems like a low threshold, but interested what others think and do in their practice. #NephJC
First things first β where the story starts!
Time to unpack the 4-hit hypothesis of IgA nephropathy β understanding each step opens doors to new treatments. #NephJC

Hi π. I'm Tiff Caza, a kidney pathologist and #NephJC fan! Looking forward to learning about IgA. No COI. Hoping to see some of you all at Kidney Week next week π
There is a 13-year gap between the approval of new drugs for use in adults and their approval in children. Could extrapolation accelerate clinical trials in pediatric nephrology? Read more in #ASNKidneyNews kidney.pub/KN1710-10
I agree! Super cool. I learned about this at Kidney week in 2022 and signed up with the NKR after. It's a good insurance policy to have a voucher for yourself and great you can list family members too (my kiddos have vouchersπ)
Did you know inadequate renal biopsies are on the rise?
Recently, Dr. Shree Sharma and @glombandit.bsky.social sat down with Dr. Vandana Niyyar, @nephrowhitt.bsky.social, and @juancarlosqvelez.bsky.social to discuss renal biopsy adequacy and understand why it matters.

Thanks for hosting us @kidneyboy.bsky.social ! π
It's not just TB. . . I think this is still true for a lot of conditions, such as psychiatric diseases, epilepsy, HIV infection, etc #NephJC
That is a crazy high number!?! #NephJC
