💬 Invited Commentary: The DEKS trial showed that donor diabetes did not compromise 1-year DMEK graft success or endothelial cell loss, supporting broader use of diabetic donor tissue to help address corneal tissue shortages.

JAMA Ophthalmology. RCT: Donor Diabetes and 1-Year Descemet Membrane Endothelial Keratoplasty Success Rate. Population: 466 Men, 631 Women. Intervention: 1421 Eyes, 1097 participants. Findings: Graft success rate. Donor without diabetes 96.3%, Donor with diabetes 97.1%.

RCT: The 1-year success rate of Descemet membrane endothelial keratoplasty was similarly high for corneas from donors with or without diabetes, supporting unrestricted use of diabetic donor tissue.

Figure showing endothelial cell density over one year. A) Stratified by donor diabetes status, B) Stratified by donor diabetes severity group, C) Stratified by donor diabetes severity score.

RCT: The diabetes status of cornea donors did not affect endothelial cell loss or morphometric parameters one year after Descemet membrane endothelial keratoplasty, supporting the use of donor corneas from people with diabetes.
