Identify pathologies, vision loss source before cataract surgery WASHINGTON — When preparing for cataract surgery in patients with retinal pathologies, ophthalmologists must be sure to set reasonable expectations and modify the surgical plan accordingly, according to a presenter. At the American Society of Cataract and Refractive Surgery meeting, Jessica Ciralsky, MD, said that the first and most important step in managing these patients is to identify the cause of the vision loss. “Is it the cataract? Is it the retina? Is it both? Sometimes this is really difficult to know,” she said. “And so, I think setting reasonable expectations is so important for

Re: Zhang et al.: Rates of endophthalmitis in prefilled versus nonprefilled syringes for intravitreal injections: a systematic review and meta-analysis. (Ophthalmology Retina. 2026;10:165-175) We read the systematic review and meta-analysis by Zhang et al1 which compared endophthalmitis rates between prefilled syringes (PFS) and glass vial preparations (GVP) for intravitreal injection and commend the authors for addressing an important topic in clinical retinal practice. However, we also have several methodological concerns and believe the clinical interpretation of their findings warrants further discussion.

Foldable capsular buckle as a viable intervention for retinal detachment in late pregnancy A 28-year-old female patient who was 36 weeks pregnant reported experiencing floaters. Fundus examination revealed a horseshoe tear at 10 o’clock position, a resultant retinal detachment extending through 2 clock hours and macular unaffected. Typically, scleral buckle is suggested but the surgical procedure - pulling of the rectus muscle - may increase the oculo-cardiac reflex as well as intraoperative and postoperative pain1.
