The biggest reason for longer waits is staffing shortages, which are driven by more people leaving healthcare practice than entering, esp nursing, due to bad management practices (often related to private equity).

This administration’s immigration policies will significantly worsen these shortages.

Biological plausibility is essential here. The SIJ complex is among the strongest in the body, moves <3mm under physiological conditions, and no clinical testing determines abnormal movements. We need to be clear with what we mean on this topic. “SIJ dysfunction” needs to 🪦 #spine

SIJ dislocation or fracture, like that which may happen in serious trauma (60mph highway crash), is determined on imaging and is unrelated to the kind of SIJ issues seen in outpatient clinics.

We need to stop saying “sacroiliac joint dysfunction” as this implies “abnormal alignment or movement” (Dixit, 2025). This is not what we see.

Instead, SIJ pain (outpatient) or in the few cases where radiological evidence +/- labs support, sacroiliitis, or fracture/dislocation.
