If he thinks that image of him in robes emitting light beams from his hand is that of a doctor, he's very confused. But also, how would it be sane for the president to post an image of himself as a doctor?
If he thinks doctors dress like that, and have light beams coming out of their hands, he is seriously mentally unwell.
His "excuse" that the image was of him as a doctor hasn't made things any better--if he thinks that a doctor dresses like that, and sends light beams from his hand, then he is seriously mentally unwell.
