1/10 Last #NephJC we discussed Pegcetacoplan in C3 Glomerulopathy and Immune-Complex MPGN #TenPostNephJC
Given the central role of complement dysregulation in C3 glomerulonephritis, we assessed whether complement-targeted therapies confer therapeutic benefit.

Excellent job @glomerican.bsky.social 🤩🤩
Maybe as an early option for biopsy proven recurrent with proteinuria, declining egfr and active lesions. Not as first line though for low grade proteinuria. #NephJC
Let’s keep in mind that VALIANT had less than 10% history of transplant #NephJC
We need to see longer follow up recurrence, progression to ESKD. Maybe sequential or switching for partial responders or intolerant but still food for thought #NephJC
Yes and because it was hierarchical design they couldn’t get conclusion for the other outcomes - staining #NephJC
Even if they started with lower UPCR in the placebo, the shift is good in intervention vs placebo! #NephJC
Change in activity score didn’t differ though 🧐 #NephJC
Good point!!
Me too I was surprised too #NephJC
Pts with intervention had lower egfr and higher UPCR 🧐🧐 #NephJC
Not all completed the open label period 🧐 #NephJC
Not in Peru at least #NephJC
But not for the adolescents though 🧐 #NephJC

7. Personal level. Beyond his talent and expertise, I met an extraordinary man. Humble, altruistic, wise, friendly, and with a great sense of humor. I brought him a vacuolar cast tie as a present and he wore it proudly right away 🤩
6. Prof. Fogazzi. Definitely a big highlight for me was to meet for the 1st time Dr Giovanni Fogazzi, an icon in the field. Still remember when I bought his book early in my career. One my fave pics is this one that captured a moment when he stood up to ask a ? after my talk. Very cool. #BigFan
1/n Recently attended an international congress about Urinalysis in Santiago, Chile 🇨🇱. I was expecting a small meeting. I was wrong. Pleasantly surprised with the size of the meeting and the energy and interest from the participants. Let me share the highlights of my experience:
Day 1, #kidneywk 25, We attended the poster presentation, which focused on an important topic: Reducing gaps and strengthening training in urinary sediment analysis. We were joined by someone passionate about U-sed. @juancarlosqvelez.bsky.social , @roxnonna23.bsky.social .
Problem here is - especially in places like ours (India), affordability remains a problem - with the amount they spend on a biopsy they could afford 6 months of non immune therapy - so our biopsies are probably- At diagnosis and then only when absolutely indicated

The 2025 @kdigo.org IgAN guideline quietly redraws the map: new drugs, new thresholds, and a sense that the era of “supportive care only” is finally over #Nephsky #Medsky

Great chat Saivani 🤩
Fascinating finding: many patients lost chimerism but stayed IS-free. 🤔 This suggests chimerism is a transient 'switch' to turn on tolerance, not the long-term mechanism itself. We urgently need to identify that mechanism for safer weaning protocols. #NephJC
