update - I 'freed' the knuckle returning it to normal over 2-3 months of gentle stretching and moderate twisting.
It felt like a broken knuckle one day for now reason - arthritis hard! stuck and frozen - I've had frozen thumbs that took 5 years to fix.

I smoke weed, so I tolerate a little cough - but when I add humidity into the house it goes away.
