Do you prescribe hydralazine?
Or, do you 🛑 it as soon as you see it?
Hydralazine persists despite TID dosing & risks like... vasculitis 🔥
Are you asking Why-dralazine?

Consensus
“To treat or not to treat?” is a profound question in asymptomatic pts.
The authors conclude that the trial does not suggest “do nothing” for mild/mod ⬇️Na, but rather that intensive treatment does not confer clear benefits in this short study.

Question 2/3
#NephJC #NephSky
2. Do you think if strategies were followed longer (60 days post hospitalization) that the outcomes may have been different?
3. Would you want to see an exclusively outpatient, protocol driven intervention with outcomes?
6/10
Question 1
#NephJC #NephSky
1. What is your treatment of choice for a hospitalized, asymptomatic mildly hyponatremia (128-134 mmol/L) euvolemic pt?
a. nothing
b. fluid restriction
c. urea
d. vaptan
e. other

A figure paints🖼️ a 1000 words!
☠️Death occurred in 8% of both groups
🚑13% of intervention & 14% of control pts were rehospitalized
📆The median length of stay (7 days) was similar in both
There were no differences in cognitive assessment, QoL or rates of falls at 30-days
4/10
1 post results
🏥At D/C 56% of intervention & 37% of control pts had normal Na levels
🧮Estimated absolute difference -1.3% (95 CI, -4.9 to 2.2; p = 0.45) b/t groups
⬇️Na recurrence or persistence at 30-days was approximately 41% in both groups
3/10
1 post methods
This is a pragmatic, RCT, parallel-group, multi-center superiority trial with blinded outcome assessment.
Conducted at 9 centers in Europe from 8/2018-4/2024.
University & community based hospitals were included.
(Sorry, can’t resist a good Na joke)
2/10
What'd I miss?
⬇️Na is associated with osteoporosis, falls, fractures, gait disturbance & subtle cognitive impairment. Among patients with mild/moderate hyponatremia it remains unclear if normalization of sodium levels is beneficial.

What do you do with a consult for a hospitalized pt with Na = 131?
“Just chill” 🥶or “Guns a blazin’” 🤠(fluid restrict, urea, vaptan)? Do these strategies improve mortality or re-hospitalization?

🧂Hyponatremia. We see it. We fix it. #NephJC
But what if fixing the sodium… doesn’t fix the patient?
