This is what we found. Sedation (often in elderly non-starved patients) is less safe that a well controlled GA and some vasopressor. It also makes the whole procedure much quicker. We heavily protocolise it (ideal drugs to use and haemodynamics to aim for) to help the Registrars out of hours.

I will add that acute strokes can often be Aphasic and/or have some degree of drowsiness or confusion which can mean they struggle to follow instructions and sudden movements during the procedure confer significant risk.

As someone who does a lot of thrombectomies I would think some of it comes down to better cardiovascular stability in an acute stroke compared to an active STEMI. AF is the most common problem and we usually want reasonable rate control before GA. Generally the procedure is much quicker under GA.

Today was a busy ITU day where a lot of work ended with the same net number of tubes as we started with. Just… rearranged.

Also it turns out you can just buy the Valentine’s Day Colin the Catepillar and no one checks if you’re in fact single and going to eat it on the 12th.
