Had a patient with an ECG that shows high lateral ST elevated myocardial infarction. It's a heart attack that stops blood flow to the first diagonal artery supplying a part of the heart. We call this South Africa Flag sign as the flag patterns correspond to the ECG changes!

The internal/general medicine doctor is like the most seen inpatient doctor around in the emergency department given with other being surgery generally because a lot of patients that comes to ED have medical problems like lung/kidney/skin infections, heart failure, dementia, COPD, delirium, etc
One critique I have of the Pitt is that they should make use more of inpatient specialty doctors coming down to ED.
ED doctors don't do everything, they assess, procedures, stabilise and refer to inpatients. I have barely seen an internal/general medicine doctor going around reviewing patients.

This is why I really want to make it work for my career as an Emergency doctor. The rest of other things like self satisfaction with hobbies is fine but feeling like being a useful person or someone that's worth it is largely through my work. I don't think I'm much of an interesting guy tbh!
Honestly a bit of a bad mindset as I'm sure there's many things outside of work that should make you feel like you're a decent functioning useful human being but like.. I feel like if I don't have emergency work, I will spiral into brainrot depression and feel highly low.
I generally feel like I'm a useless person with my low level of confidence and my work as a doctor helps to get rid of that feeling. I never thought much of myself as someone who is good in much things, especially with friendships so having that work satisfaction is a bit of a craving :x
Will focus more in touching grass (exercise, piano, fishing, actual irl responsibilities, and ESPECIALLY WORK/STUDIES) and actually get good sleeping habits despite horrible shift work. This work is so important to me personally honestly.
However, as I've said many times in the past. With how I operate and also dealing with my own mental health and social energy, I'll have to cut out more and more online time now, might altogether just abandon chatting/replying in DMs at some point for long times. Will probably talk in servers.
