As I look at your example, I would probably want to substitute the term "responsible individuals" with "researchers" and reword #4. Does your ethics committee give you some leeway to do this?
1/ The term 'legal rights' seems to be used in many consent forms. Ours don't have 'regulatory authorities' (who are they, by the way?). Our generic consent form for minimal risk studies can be accessed here researchethics.ubc.ca/clinical-res....

There is definitely a tension between procedural ethics and ethics-in-practice/situated ethics. I think those of us who conduct qualitative research or use community-based/participatory research approaches often have to be quite creative. Do you use lay information sheets?

TLDR: non-motor symptoms after stroke are common across domains and guess what? Not a great deal has changed in the last decade. A real wake up call to the field to address this huge unmet need
