Diagnosis: Epidermotropic T-cell lymphoma with large cells

See large cells on the H&E (>25%) w/ epidermotropism. CD3+CD2+CD5+CD4+CD30+ with marked diminishment of CD7 and negative for CD8.

The patient had Mycosis Fungoides with large cell transformation. He passed within a few months of this bx.

Diagnosis: Eccrine carcinoma, syringoid type
Rare (~60 cases). Arise on H&N as large nodules/plaques. Infiltrative dermal neoplasm with biphasic pattern: superficial squamous nests + deep infiltrative cords with ductal diff. in desmoplastic stroma. IHC: CK+, EMA+, CEA+ highlights ductal structures.
