To me, the “dereliction” in McMaster’s book was the chiefs’ failure to speak honestly even to McNamara and LBJ. They were too focused on protecting their budgets, jobs and service priorities. The “dereliction” was a failure to do their job. It wasn’t necessarily a call to resign.
