Agree with assessment that a global organisation would be best positioned. For #kidneycancer we built a global search, updated every 24 hours. But clinicaltrials.gov API changed, crawler code no longer worked. It's a huge undertaking. Any success stories out there? Pan-national.

2. Trial descriptions should disclose dose and schedule information of all drugs involved. Have seen this withheld by manufacturers as "competitive information". How does a patient weigh up toxicity, #Qol without knowing?

Couple of additions to your frustrations from another non-US patient:
1. All trials should list locations in real world terms, not manufacturer secret language such as "site 21". We patients need to see which major cancer centres (and investigators) are participate. (1/2)

Agree, you shouldn't.
But even in countries with fully funded universal healthcare, we see these GoFundMe campaigns. Often for non-evidence based care, alternative treatments in private clinics in Mexico, Germany. Sad to see.

Thanks for sharing. Beautifully written. The term "chronic terminal" resonates with many of us living scan to scan with stage iv cancer. One foot in each world. Many of us in uncharted territory.

One practical recommendation would be to follow respected international guidelines. Eg., for #kidneycancer the European Urology Guidelines which have a rigorous evidence-based process, also EMA (vs FDA alone). Look for variances etc.

Especially in the US where DTC (Direct to Consumer) drug marketing is rampant... one of only 2 countries where DTC is legal. The other one, surprisingly... is NZ.
Kudos to @htaiorg.bsky.social and the Patient and Citizen Involvement group (PCIG), book editors and the many contributors. This is quite the comprehensive volume on #HTA with many global perspectives.
