I Almost always reduce what they are on. Between the natriuresis, nausea and the reduced oral salt intake,the requirement for diuretics goes down significantly. Even in heart failure patients ,dose comes down.Haven’t had much pre renal issues since I started being proactive in dose/med reduction.
